Can you explain what is happening on the back of your head design of the guy sitting in the toilet?

I can suss how to make the front, but cannot see how to get the neck out centrally at the back.

it is just a tuck under the chin. a small bubble then tuck the bubble in under the neck. The pressure of the neck and hair hold it together
